What to do next after website getting rank 1 ?

You can target related keywords. Also if it is by chance I mean if you just got lucky then try to implement your competitors strategy to keep that positions.

and don't make a lot of changes at the same time. You might drop the ranking if you do so.
 
Creating second sites using same keyword may be smart approach but you should avoid duplicate content. Use different title focusing on main track. If you select absolutely different niche, it may have chance to decrease authority. So use relevant and unique title for better ranking.

If you don't have engagement with rank 1, it means your keyword doesn't have traffic, so there's no point getting the 2nd rank too, because it won't have traffic either. Better focus on a stronger keyword.

so you say that we can't create engagement for that keyword ?
imo , im not sure we cant do engagement for low keyword with no traffic ( im sure someone call it "branding" ), i just need to learn another thing for that.

So you have a low volume non-commercial keyword and you are ranking nr.1
I wouldnt even think about monetizing this. You shouldn't either.
Focus on high volume or commercial keywords.
But then its difficult to be nr.1? I know ;)

You haddent told that yet. With that volume nr. 1 would get you a real big slice of the pie.
In tier 1 country millions of visitors should get you a monthly check for 1000's of dollars for showing ads.
So it is commercial !

There may be a language barrier, but by engagement we understand clicks. So it's ranked in 1st place but no clicks, right?
 
This story is very strange.
Nr.1 with 7.2M volume and no clue about SEO is strange.
