To keep your ads effective, use multiple accounts, multiple images and content, use one website with multiple alternate urls, use multiple campaigns + separate fan pages. This way, your cost per result will be kept low and avoid competition with each other and more potential customers will be reached.

It's already been few months i am facing this issue, during August was the best performing. Then after October everything went downfall, until today.

I think you could try changing your content. Since your account is fine, you may have ignored it, but it makes your content less attractive to potential customers. There may be a trade-off between attracting customers and having your ad account restricted.

Change the ad content, change the target and see.
 
It could be your landing page or ad copy, if you target words like 'free' you won't get any paying customers etc

Instead of covering the entire country, you should focus on big cities so it's easier to find potential customers.
