BlackHatCodeBreaker (BHCB) anyone using Geo Targetting and how?

I really need help with this, all my CPA offers get redirected to SurveyScout and they requiere users to enter a credit card, even thou i have a good one set in place.

talks about making BHCB geo targeting capable, the problem is that when i import the CSV database it gives me an error "missing field"
I need a dump of that database on .sql format, or an explanation how this is really done.
