Everything is explained in the guide - how to monetize, which affiliate programs to use. It is perfect for newbies.
No other costs are needed. There's only 1 thing if you want to become an Amazon Affiliate. To apply you need a website. Either you will need to build it with money (domain+hosting) OR you can apply with a free website like Wordpress.org or Blogger.com.
But Amazon is not mandatory. There are many affiliate programs suggested in the guide.

1) Yes, you can create the accounts for all these countries. Check out this guide.
2) Yes, you can use almost any affiliate program. There are plenty of examples in the guide.
3) Can't say how much it's possible to scale. Best I did was $8K a month with this method (4 affiliate programs combined), but I did not spend more than 2 hours a day for this. But it will take a lot of work and you can do that only by promoting several different affiliate programs, not only Amazon.
