Wordpress Code Verification plugin????

Does anyone know or can help me find a plugin or some kind of site with a code, i basically want to have a 1 page site with a field to put in a 10 digit number that i have listed in an excel spreadsheet (ofcourse i can list those numbers wherever there needed) as long as when the number (code) is entered and they press "SUBMIT" it will take them to the next page.

Is there some kind of Verification Code Plugin for wordpress, please let me know.
