What's going here ? Big G broken ?

I was just checking several of my domains and it seems something is very wrong with the SERPS. Instead of pulling the title, it's showing the main keyword for one query and the real title for another query.

Same of meta, for some of my articles it's showing picture alt for meta. Same experience for some one else ? So f*cking freaky....

For one site, I see a title I changed months ago...

For another domain, I see the title of the domain it used to be redirected to months ago as well.
