Brilliant method mate. To the people, who are asking for an example website, the OP already mentioned that he never implemented content locker into his website for the reason of being banned from PPC networks. He sends the locker link to the customers via email.

Which email provider to use
The only problem I see with the method is the cpc vs the payout of most zip/survey offers. The cost the drive traffic would be much higher than the revenue unless offers pay in the $20-50 range

Interesting method, but some things I do not get at all.
Only COD option, so that means if somebody redeems the coupon only payment method that could be used is COD? I do not see how this e.g. would work with Shopify. I have not seen that their coupons have any option to limit payment methods.
"After they enter their address and click purchase, redirect to out of stock page". Again, how would you do this? Then the item must be already out of stock on product page. You cannot have stock and let people add to cart, then on cart it says sold out. Which shopping system would be able to do this? Shopify not, Woocommerce not.
Also, so that means that the voucher issued after doing the CPA offer is only for one certain product and that is 80% off? Because if it storewide discount it would be even ten times more complicated.

Idea is great but how you generate an automated coupon after they sign up for the offer?

I've thought of something like this before but didn't really bother to go ahead with it because I didn't want to kill my email list. This will work but you're gonna need 2 things -
1. An active email list
2. Good products that'll actually make people want to get coupons for
Another twist - Do a product giveaway, tell them to do human verification aka complete an offer on your locker to confirm their entry.
