Submitting Data to a PDF from via HTML and/or PDF forms

hello i want to keep registration in my website
the registration form should be in pdf format and data also stored in fdf or xfdf format and readable in pdf format,
and also how to write program from HTML - PHP - MYSQL

step by step process can anybody help me out in this.
i am severely searching for past 3 days unable to get this scrip/program
help me guys

brother thanks for the helping me but i need pdf form on website to fill my clients and the database should store in pdf format and readable

Mmm, a form in a pdf?
Then you would require more than just html, you'd need a database.

Perhaps you can try "fillanypdf". It seems to provide what you want to do.
