New to CPA .. want help in creating compaigns ..

I am trying to create a new cpa campaign on V Commission n/w.
I am not exactly sure how to do that.
There are fields like - 'campaign desc', Leads, Rate, campaign category, approved subject etc.
Can some show, with the help of screenshots, how to fill those fields and what they mean.
Any help is appreciated.
