The job would be to create data entry forms.
Also validate forms before user submits them.
Also your code should be able to send emails if required.

Database: MySql
Language: PHP

I'm also a programmer, so I can help with architecture and database design. But I cannot help with PHP as I'm a .net developer.

Also make sure you will be there to provide after sales support for bug fixing if there are any.
