AdSupply have given me good rates with OK quality ads. Their offers aren't spammy and there are multiple ad placements available. US traffic gets a great eCPM and local traffic from here in South Africa is also fair. I found their rates far better than PropellerAds.

There are a lot of banner CPM Ads Network for small publishers out there, the issue is which of them are really good and worth trying out? many bloggers have listed someone of the those Ad networks which you normally find on many of the blogs but is just sad that majority of them are only promoting their affiliate links with those Ad Networks. that being said, am not saying most of the listed ad networks are bad but the bloggers are mostly listing some of them because of gain they get from them.

have you considered looking into adsense i get roughly a $3 cpm with them

Would you guys prefer to monetize with only CPM Ad Networks or both CPC and CPM Ad networks, cause it looks like most publishers are focused on CPM ads these days.

You should try some banner networks to work with.
I can suggest you trying adtarget.io. They are good at it
