As far as I know, Instagram was suspending accounts that were inactive or that met certain adjectives which indicate that the account was inactive for X amount of time. When you log in to that inactive account, the follow is returned to the pages you followed. If no one logs in for a long time, that account is completely deleted.
I was reading your answers and you say that the followers are 100% organic and you never bought or were bought by bots. Maybe the followers you lost went through this that I'm explaining to you.
