I think one post nailed it. IP need to be close to business area. No proxy. No VPS. Natural residential and/or biz IPs. I'm also noticing the same now with G+ reviews. Seem to being filtered more now... PM me if you'd like to exchange Yelp and/or G reviews for your biz.

So if someone who frequently travels opens a new account and starts posting reviews for high quality restaurants they've visited, their reviews won't stick because they look like they were paid to give a review? I understand these paranoid companies are just trying to filter out fake reviews, but that's terrible if Yelp is doing that.
