6. What is the next step? What am I required to do to get my funds?
Section 107(4) of the Domestic Banks and Financial Institutions Act requires the Liquidator to issue a Notice, by 29 August 2018, to all depositors, creditors and lessees of the Choice Bank Limited to file a claim with the Liquidator. The Notice will inform creditors of the procedure and deadline for filing a Claim.
The Liquidator intends to issue this Notice before 29 August 2018.
By law, the Liquidator must allow creditors a minimum of sixty (60) days from the date of the Notice to file a claim.
I advise everyone to contact firstchoice again just by making sure that you actually recieve the notice till the 29th of August, so what i did was the following. Sending an email to
[email protected] with the following details:
Name:
Address:
Email Address: (please use one you check frequently)
Account number: (your account number with Payoneer/First Choice)
Amount owed (and screenshot attached with firstchoicepay that shows that balance)

On the 28th of August (or before that) every FCP customer SHOULD recieve a form where you can file an actual claim, this has to be done within 60 days (october 29)

Recently Choicebank updates their website with certain claim forms that we can fill out. Before you fill out any of those forms to claim your money, please read general instructions:

So your todo in order to claim your money is:
(a) downloading and completing the appropriate Claim Form:
(i) Claim Form 1 – Secured Creditors
(ii) Claim Form 2 – Unsecured Creditor: Depositors
(iii) Claim Form 3 – Unsecured Creditor: General Creditors
(iv) Claim Form 4 – Unsecured Creditor: Prepaid Card holders
(b) attaching the applicable supporting documents to substantiate your claim, and
(c) submitting your completed Claim Form and supporting documents by 29 October 2018 to the Liquidator for Choice Bank Limited, by either method below:

For people that have funds frozen in their FirstChoicePay Account please use Claim Form 4.
Found information on another forum that Choicebank will possibly create an automated claim form submission on their website where you can upload the claim and all the documents needed. It also should include a status check on the form that you have submitted. So at the moment i don't recommend filling out the form right now.
This situation is bullshit, i can't get ahold of anyone at FirstchoicePay neither ChoiceBank Ldt. - People in other forums claim that the stuff has to be notharized. Seriously, thats a load of bullcrap. I'll call them tomorrow and ask them whats right and whats wrong about all this stuff. After that i will just fill out the form.
Make sure if you file your claim that you download ALL of your transaction reports from FirstChoicePay (site is only up until October 1st) - Also take a picture of your balance in your account.
Back again with this neverending bullshit.
Just a couple of minutes ago i sent them the liquidation eMail. I sent the following stuff:
-notharized german ID Card (you can use your id card or passport, but it HAS to be notharized)
-Claim Form 4 filled out
-FirstChoice Registration eMail
-Payments and Transactions of new and my old card (you can find those in your firstchoice pay account)
-a .jpg file of my firstchoice pay account overview ( so they can see how much money is in my account
The current situation is pretty fucked up, i mentioned in my email that i want to have a confirmation email of them recieving my documents, else i will send this email to them every 3 workdays. (Not trying to be an asshole about it, but after 4 - 5 months of just straight clusterfuck i am sick of dealing with them) - I suggest you mention the same thing in order to get a confirmation email.
The Claim Form 4 doesn't have any fields that you can fill out by computer, so i uploaded the file that i used to fill out Claim Form 4 (you can type into certain fields) - However, make sure that you sign this form with your real and original signature

This situation came to light in April and you had plenty of time to withdraw your remaining balance, which was aleady loaded to the card, so under $10k.
