Frankly speaking, have no idea. They say that bing doesn't allow any VCCs, so I haven't even tried. If you find a good solution, share it with us. :-)

I use Bing cus it's cheap and drives results. For banner advertising I use sitescout cus there are lots of targeting options. I've tried ppcmate, but it's like dntx -- low quality bot traffic. So, at this point I can't advise you something better than Bing.

I don't think it is. For example, you can earn up to $3 for 1,000 impressions on adsense banners (if you have more banners, it'll be more). To get those 1,000 impressions, you need to get your ad clicked at least 1,000 times, and even in the cheapest rates it will be $10, so you will be lucky...
