No, it doesn't include domain and hosting. You can get a domain for like $5-10 per year. You can get hosting for about $5-10 per month.

The typical workflow is like this:
-After placing the order the submission team will research for keywords and domains proposal
-After keywords and domain are approved, you purchase the domain
-The submission team will ask you to provide cpanel or to install wp on purchased domain and then to provide wp admin
-The submission team builds the site template, articles, etc.
