There is an issue of trust here. How the hell would i know if all the links has been placed? You only provided screenshots and only 30% of them. And now i found that your site are not even index at least 3 out of 5. How would i know for the rest of them?

Give me the full list of site. I will get them index myself and do the rest of ur client a favor.

The keywords you submit will be used as the anchor text. Yes, you can do that, just make the instructions clear in the order form.
