Hi,
In an effort to maintain a culture of authenticity on the site, Facebook requires users to provide their real first and last names. Impersonating anyone or anything is prohibited, and fake accounts are a violation of our Statement of Rights and Responsibilities.
Our systems indicated that your account may not be authentic based on a variety of factors. If you believe you were disabled by mistake, please fill out the correct contact form to help us verify your identity. You can do so by going to the following link:
Please make sure to upload a government-issued ID with this report and make sure that your full name, date of birth, and photo are clear. You should also black out any personal information that is not needed to verify your identity (e.g., social security number).
If you do not have access to a scanner, a digital image of your photo ID will be accepted as well (i.e. a picture of your ID taken with a digital camera). Rest assured that we will permanently delete your ID from our servers once we have used it to verify the authenticity of your account.
Unfortunately, we will not be able to process your request unless the proper identification is attached to the form. We apologize for any inconvenience this may cause.
