New accounts getting flagged fast because people rush the warmup period. Geographic targeting being too broad or too narrow. Using obvious gambling-related keywords even in cloaked campaigns. Not having proper backup accounts ready when things go south.

But honestly, the biggest issue is usually poor landing page strategy. You really need to understand your funnel from ad to conversion, and have multiple backup approaches ready.
