Why do google reps don´t care if you run BH ads? I have been contacted by Google staff many times who wants to help with my account(s). Its clear that they see that its BH, and its clear that there are alot of spending. Still they let me pass.

What are the best ways to minimize click cost ?

Target medium and low traffic keywords.
Use long tail keywords.
Best match your keyword to ads and to landing page.
Try using single keyword ad group campaign.
Above points will help you in minimizing the click cost.

What if my account have over and over communicate "we cant determine" or "keyword pending review'' and after that accounts suspended after some days without results? What can be a problem? Account warmed for 72 hours, etc.

Ah, I guess, i missed you question.
Well, are you advertising something which is against Google policies? This is the reason of suspension.
We cant determine - For new accounts, just after adding keywords, Google shows this or "pending review" status. Once they pass the review process, it will be active and live. If not, then Google takes abuse of its services very seriously and suspend the account immediately.

Google simply don't allow to advertise the site which is not as per Google policy. Google representative helps you to avoid/remove or suggest to make changes in ads/content/landing/images that may cause the suspension and if you follow them, they do help and re-activate your Google ads account. But they do not help for products which is directly against policy.

You can start with a simple campaign using broad match keywords with negative keywords and target the area that you serve (make sure you select the exact radius where you will be serving). Start with some $30 a day (divide the budget in all your campaigns) and increase it gradually as you get the profit. Keep running for at least 45 days to measure result.

I have some questions, would be great if you can answer. I am working on treshold accounts.

1. What is best time to warm accounts, 2 days, 3 days or more?
2. Starting campaign with budget $100-$200 make sense? I mean is it a chance that this account will not be suspended and will work properly with burningn treshold in 1-2 days?
3. When I increase budget, account will be under review?
4. I have problems with my accounts, many of them got "keyword pendind review" and they stop working or are suspended. How to avoid it?
5. There is the best campaign format? Leads, web traffic etc?
6. If I am creating account in USA with trshold, then I am creating campaign for other country, example target France, is it to risky or doesnt matter? If yes, what do you suggest?

1. There is no best time. Some people do it for 2 days, 3 days. Usually, it is good for at least 1-2 months.
2, Yes, that's perfectly fine. Some times, you won't be able to use the threshold amount and the account gets suspended. There is no fix strategy for this.
I have seen a US based 2 years old account where ads have been ran for 9 months spending $30k monthly and then the account got suspended.
It could be because Google started analyzing old account. Usually Google don't scroll the old account qucikly.
3. Don't increase huge budget quickly. It will go under review. Play smart here.
4. Stop using those keywords. Use broad match modifier with keywords, they work pretty well.
5. No format. Usually people collect leads and then convert them to sales. They work better always.
6. That's not risky until you promote a offer which is not allowed in France (or which ever country you are targeting)
This is perfectly fine. You can go for it.

Yes, the algorithm changes immediately and start to format the campaign/adgroup within the new budget you set or the money you add.
