Body Fat Calculator "email collecting"

Hi few days ago i saw fitness website with some kind of contact form.

It was something like:

Your weight _____
Your height ______
Gender ______
Your email adresss

<send results>


Is it plugin guys? Or how can i find something like this i think its great for health/fitness niche.

I dont know if it was body fat calculator but it was something like this.

It can be done with a little html+css+javascript code.
If you don't know programming, you can look for a customizable wordpress plugin, but you should implement the calculation formula by hand.

I'm sorry, are you tried to compile the form for a reverse engineering study? The result is viewed on the same page or sent by mail? Because if is sent in the mail, you don't have need of customize form.
Anyway it's very easy to implement. Look here:

Thats awesome! results can be showed on the page or send by email i dont really care point is that i need to collect email adress.
So yea i will need some java guy that can do this for me so i can implement it to my wordpress blog.
