Just checked. It's gone for me too. Did you submit yours before it disappeared? If not, there's an alternative but you have to jump on it asap. Go to type "/mentions" after facebook in the address from your iPhone in Safari. I cant put urls here lol (method only for ios). Scroll down and download the app. Open the app and type in your name and click the option "My page isn't verified." Type in the web address to the your facebook page. Give them your drivers license or articles of incorporation. Also put down you official website. Cheers!

This is an official app from facebook btw. They released it today.

my page is community/ publishing page 800k likes i did the fb mentions thing and put the link to my mylikes gallery do you think it will get verified?

Dont do that you will end up resulting in loosing your page

so. i checked all threads and i read a lot of new forms etc for verify pages.

i am a designartist but i saw some pages in germany with verify badge too. so its possible for artists in all genres.
i have created an official website but i link it to my fb page directly. ist it ok? so you cannot see another page and it jumps to facebookpage. i write down the official info mail to my fb page and filled out things like address (but without street name).
i dont use my real name but i have an artist name in my id card, too. and facebook knows my real name because of the ad service. i wrote them sometimes and i think it doesnt matter if i use an artist name.
 
I found the way how to verify pages. Already verified 2 and the 3rd to be verify soon. pm me i might help you guys.
 
i searched over net and didnt find any way to verify pages. it only can be done by fb staff, so wait what else!
 
maybe the link work only i account how do some ads before !
