Which Plugin Can Create This?

Please I will like to know how to create this sort of form or questionnaire in the image below. The option the user choose will determine what the next question will be and all the answers can be generated.

Could be done with simple php code, and a bit of JS. It looks custom to me.

Yes, it is custom. I'm asking if a plugin can do this on Wordpress or maybe a simple php code

Its also the kind of form used on loan sites, whereby the option and the response given by the user will determine the amount that will be shown to the user. Is there a plugin that can do that?

Well, try the plugin Deleted member 15724 mentioned about. If you are looking for custom script, obviously it is going to cost you money. Try posting in the hire a freelancer sub forum.

Gravity forms does this. It’s called conditional logic.
