Please I will like to know how to create this sort of form or questionnaire in the image below. The option the user choose will determine what the next question will be and all the answers can be generated.

Well, try the plugin Deleted member 15724 mentioned about. If you are looking for custom script, obviously it is going to cost you money. Try posting in the hire a freelancer sub forum.
