Twitter ads are still pending

Have you ever encountered a situation where Twitter ads are always in the pending status? My account is 5-6 years old and I have passed the $200 threshold through VCC, but after I created and published the ad, the status is always "pending". Does anyone know how to solve this problem? The ad does not violate Twitter's ad policy.
