Op, it would be interesting if you can pin point some blueprints as to why some URLS won't get indexed. For example, looking at the tables from Linkdexing and LinksIndexing, clearly URL #2 is not getting indexed. It would be nice to do some analysis and see what were the points that trigger it not getting indexed.

This is probably something I will look at in a couple of weeks time if it's still not indexed.

Yep, I've done nothing to them at all. To try and make sure that the pages are completely orphaned, no way for big G or anyone else to get to them except through the indexing services.
