For example, when people run IPTV ads on Facebook, they don't state provider names, nor do they use terms like IPTV.

Get creative. You'll get through approval. Always have additional accounts on standby, though.

If it is a facebook app like the one above, then there are not many problems, this stuff is all over the place.

Externally hosted apps or casino websites are rarer and those pages tend not to last, and have high ban rate. But you know that banning never stopped a determined advertiser:)
