Serious advertising violations on facebook

You're probably being too direct in your approach.
Review your ad copy and creatives. You can say what you want to say to the consumer, without being blatent.
Think about the language you're using etc.

For example, when people run IPTV ads on Facebook, they don't state provider names, nor do they use terms like IPTV.

Get creative. You'll get through approval. Always have additional accounts on standby, though.

Externally hosted apps or casino websites are rarer and those pages tend not to last, and have high ban rate. But you know that banning never stopped a determined advertiser:)
