'request a call back' form on wordpress

are they any easy ways to set up a form on an existing page... I need a form right at the end; so i can get the persons email/phone number.

Do you mean like a page displaying your contact information?

No... I have a sales page and at the end of the sales page I want to add a box that allows someone to give me there email/phone number to 'request a call back'

That's the one I'm trying right now unfortunately its a side bar widget I need something at the bottom of the page.

This can be done with any form plugins such as Contact Form 7 or Gravity Forms. If you don't know how to set those up, feel free to send me a PM. We do this kind of stuff for our clients every day.
