Hosted account simply refers to Google AdSense verified over their native products. For example, getting approved AdSense through your YouTube channel, blog established over blogspot.com etc are hosted accounts indeed. On the other hand, AdSense account got approved via custom site made over WordPress, HTML, etc. are non-hosted accounts. Hosted accounts can't run ads on custom domains. For example, your AdSense account approved via YouTube channel can't show ads on established .com WordPress or such custom sites.

Earlier, one non-hosted account used to run ads on every other domain without applying for AdSense from scratch. And now, you will have to apply for AdSense approval for every single domain hosted on servers other than Googles'.
