Potentially Dangerous Site False Positives

Do you know what triggers the "potentially dangerous site" alarm on Firefox or Chrome browsers? I've been to sites that are obviously safe sites where this trigger comes on. Is this a coding issue or client side issue? Do you know?

I'm going to go out on a limb and say visiting a potentially dangerous site would do that.

More seriously, sites that are known to serve malware are generally flagged by Google and friends and added to a shared database of naughty sites.
 
Many of these are because the malware has been found in the code and you will not necassarily see the issue yourself. It could have been injected into a template which is very common.
