I don't use them much anymore, but my largest monthly payout was a little over $20k. At the time I was their biggest publisher, but no idea about the environment now.

@dnreg, can you tell us how much % of your leads were blackhat (if there were any)?

None of the leads were blackhat actually, all leads were legitimate. I've never tried to cheat CPA gateway companies to be honest.

Regarding what company I use now, I used BHCB for a while but am having better results not using them these days. I don't want to divulge publicly which company I'm using but will chat in more detail in PM if you like.

SEO, all organic traffic so it converted/converts nicely.

EDIT: btw, google has picked up on si pi ay lead now just fyi. My rankings on searches started going down, then when I changed to BHCB (and later to another couple providers) my rankings started going back up again.
