I have this problem during my first try but you will be taken AFTER you have paid, just click the button given if you are not automatically be directed.

Didn't trace it but you could just click "back" until you go to the "paid successfully" Paypal page. In there, you got a sentence that says, go back to "email of Azguru". Click that and that is the form.

Another Review (I will focus more on the quality of the blogs):
I received two different reports. One for the smallest one and the other for the biggest one. I checked them together and separately. The biggest one has a unique domain count of 7% - 9%. The smallest one was around 20%. It's amazing that the disparity isn't that wide because most of the 100k blasts would give you a 3% - 5% uniqueness. Quite impressive really.
If there would be a way to raise that even more then it would be more awesome. However, if that isn't possible, this is still a great service! A+
