yes they are all unique sites. They are for domains worldwide with about 80% of them being in the US. I verify them all every few months so most are still active.

no, it's a bulk list but you can easily segment them depending on your targets (i.e. if you want lawyer's websites for example you can filter out domain names that don't include words like "legal" or "law" then you end up with tons of contact pages that are mostly for law firms or legal clinics) . I use scrapebox to do this but you can use virtually any file editing program like notepad++ for example.
