If you search "How to Twitter Verification New 2015 Twitter Verified Angela Taft" on YouTube you will find the video. He/She demonstrates the 'Twitter Account Verification Request' form.

So, the person selected 'request verification for one or a few users' and then enters a Twitter username, name of that user and then picks a category (picked Film/TV). It then asked for the IMDB profile page. Now, during the demonstration the Twitter username entered was MaiaCampbell (who is indeed verified on Twitter). The person then searches for 'Maia Campbell' on Google and instead of copy and pasting the URL for her IMDB page, copies and pastes an article about her arrest instead. And that was basically the end of the video, without submitting anything.
