I'll give it a try on one of my sites to see if it's as reliable as Wordfence.

Yes, but the developer is great and been around a very long time, and so is all of his plugins .. =)

Wordfence is fast & apt. How about Scurri , never used it but has plenty of features including CDN.

If you can afford it, Sucuri provides a complete solution. Their CDN is also good.

What I tried to far:
- Optimized the DB
- Disabled WP-Rocket (some users complain about the new version)

Didn't work

What I have:
- Cloudflare (Web core vitals > 90)
- 22 plugins (all updated)
- Image optimizing using Smush

Any idea what the issue can be?

is for sure a badly developed plugin the one that is causing that.

Easies way to spot it is to desactive the common suspects... the ones that loads in on the front end. If you have PHP my admin you can also monitor what requests are been doing to your BD and wich ones takes more time... or are a lot in numbers without need... some developers has not sense of best practives and over fload the DBs with connections and read/write requeats that cab be avoid.
It you don't see a lot of requests in the DB then some other process on the PHP is causing it... that may be mor difficult to sopot without programming knowledge
