I was running successful Facebook ads for my online Yoga and Ayurveda services targeting US clients.
2 months ago, my CPM was low, CTR good, and I was getting quality leads. Suddenly, everything died—CPM tripled, CTR below 0.5%, almost zero leads.
I tried fresh creatives, new ad accounts, but no improvement.

What could be the main reasons? Audience fatigue? Account reputation?

Meta algorithms change constantly, we can't keep things stable forever you need to change
 
review the page, if it has limitations then the effectiveness will decrease, the best way is to use less error ads

Pretty standard fare these days with niches like wellness that sort of skirt the line between personal and health-focused.
That said, there's quite a lot of factors that might affect that and the likeliest ones are audience fatigue (liek you said), factors outside of your control like seasonality or algo shifts, or a mix of both like, yeah, reputation if you've ever been flagged or have too many negative feedback since that sort of thing snowballs.
A lot of other factors and some workarounds for them you could employ. If it's real bad, you could try using a fresh B< with clean creatives. Warm up and test in parallel, see if there's any difference there.
