Looking for iPhone Unlock Website Script

I'm looking to buy a simple website script that will allow users to enter their phone's information, pay through paypal, and have the system process it. I have seen similar generic scripts sell for $20-30, so that's the price range I'm looking for. I will pay with paypal as soon as all my questions are answered.
