Is This A Theme or Plugin?

I'm trying to figure out a way to do what this lady is doing with her photos:

When you hover the image, a small description comes up along with a place to click to go to the link. I've spent about 20 hours trying different plugins and themes to mimic it, but I can't. Can anyone tell me how to do this or point me in the right direction? I would be extremely thankful.

Thanks a lot. I'd narrowed it down to that theme, but was kind of hoping it was just a plugin or something because I'm partial to the theme I'm using. I appreciate the help.

The theme is cool, but lag as hell, at least on my side.

I think it's from this "Essential Grid" plugin when looking at the class names.
Way too JavaScript heavy though in my opinion, seems to lag a bit. You could achieve such an effect with pure CSS also which would be way better in terms of performance
