If someone sends spammy links, how to prevent that?

no tool.... don't worry about spammy links.... just build powerful links, and do not use disavow tool unless you get a warning from google in search console
also don't rely an any third party site to tell you what is spammy and what is not.....

Suppose its a health site and someone sends adult links or like low links to drop my rankings, it's harmful right and it also lowers the domain rating?

You can’t. These links are out of your control. I’d disavow them even if you haven’t gotten a warning.
