This will happen with newly registered accounts. Sometimes Facebook flags it as “unusual activity” when a card is added and ads are attempted right away. Usually you just need to request a review and confirm your identity. It does not always mean you violated anything.

This is very normal. Before running ads, you should warm up the account for a few days (the longer the better). Use the account like a normal user—like, comment, share, send messages, etc.—to build trust with Facebook. After that, you can start running ads.

That's perfectly normal. Sometimes Facebook disables advertising accounts for no reason at all, perhaps because the type of account you're using is weak.

Actually, the problem might not be with your card, but rather that Facebook uses an AI system to scan accounts, so there could be errors. Please contact support to ask them to help you appeal. Account scanning happens regularly anyway.
