help to create adsense Invalid traffic appeal

Hello in 9 tis month i got adsense approval. but today my adsense closed by google because invalid trafic.
i want ask you guys for help to create adsense Invalid traffic appeal.
https://support.google.com/adsense/contact/appeal_form_adsense_admobi know most part but i do not understand tis part

Please include any data from your site, mobile app, and/or YouTube channel traffic logs or reports that indicate suspicious IP addresses, referrers, or requests which could explain invalid activity. *

how do i find this information

They are not going to give the account back. You can make a new account with new details like that of a family member.

its my second time. look like appeal only work for youtube

There is no official way to appeal Adsense ban. It is final decision which they will not take back. In very rare cases, they have given Adsense back.
 
Bump!! I know this is an old thread but I want too know if anyone has managed to get their adsense account reinstated after been disabled. My adsense account was also recently closed by Google.

File the appeal and make it as best as possible
And pray
If the ban is permanent, there is nothing else to do
