Form? What form? Google ADS campaign settings. You click on the campaign name and reach from the left tab the SETTINGS and you will see more from there.

Is the account an account under MCC ? If so, it could be a shadowban and if it is so, then all accounts in that MCC also would be shadowbanned (if MCC, check that for yourself). (This is a latest Google Ads algorithms trend going on in certain MCC accounts).

Most probably account is shadow banned. If not a shadow ban, etc, then pause the campaign, and then unpause it after 24 hours : sometimes this triggers ads to show again (sometimes).

check so that the keyword is enabled, sometimes, keywords can be stuck on review for days, just change the match type, and then change it back to whatever you wish.
Had the same problem for 2 Weeks now! After opening a ticket last week which didnt even received a reply so far, today I copied my existing campagne, deactivated the existing one and fumbled around the copied, made sure the bidding strategy is activated and on status "learning" I finally get impression and clicks.
Cant tell if the account was shadow banned or now. Not MCC. Comletly new google ads account.
