    of coz the 2nd is the best. as u having ur targeted keyword in your title.

    Sure second one cause more keywords somewhere(for instance url) is always better ;)

    Actually Google has made known to people that they can crawl dynamic pages as well as static pages.

    Even much so, if your URL is descriptive, it will be more appealing to the user.

    It matters. Second permalink structure is SEO friendly.

    Set it as %postname%.html or %postname%.php .
    Best choices ever.
