Do warnings disappear?

They expire naturally but remain visible to staff. The only time they don't expire on the date due is if you pick another one and staff decide to extend your previous one if it is still active.

Why does it not fall off the record?
Can it still hurt me in the future?
Expired ones are expired, so don't get extended.
The only way warnings can hurt you is if you upgrade to Jr VIP you are unable to open a BST with active warnings.

You're thinking way too much. Just follow the rules and the warning that expired may as well be the last one ever.
