Not sure regarding cpa lead, but most cpa companies allow you to customs your locker with the amount of offers you want displayed, etc.. and after completing the locker creation, they provide you with the html code that you then add to your index file.

I never used CPALead after it died/came back, but in every other network it's really easy to do from the dashboard, I'd imagine if you haven't been able to find a guide then it doesn't exist because it's so simple to do... play around in the dashboard or email your AM if you can't figure it out.
