Get amazon mturk requester account
Get a blog and put some content on it
insert html to automatically redirect your blog to a cpa offer after 5-10 seconds
make a hit on mturk paying $0.1 per valid hit
in description say
"US ONLY"
"1. Copy this link in your web browser - bit.ly/XXXXX" Don't put http/https as it will help cpa networks to track you. Instead tell user to copy and paste it in address bar manually in new tab.
"2. Complete the survey with your google/yahoo/outlook email"
"3.When survey gets completed take screenshot and upload it here"
"4.Get paid for valid hit"

We don't rip off CPA networks on BHW not do we condone promoting the process to rip them off (especially when they are so poorly laid out in the OP).
