many my customer face to the same rejected error affter the ads actived a few time. but they come normal when he change the page and keep the content no problem. just one of a small tips. you can try. have a goodluck to you

Changed account and profile then it seems the problem lies in the content and images you use it is not suitable, you should change and update the image as well as add a hidden link to advertise your campaign easier
