I didn't check Amazon terms, but FTC disclosure states that you should mention your links to be affiliate immediately or write a disclosure. It's recommended to write as close to the link as possible.
Example 1:
The following product <link> (amazon affiliate link) is the top selling in the category.
Example 2:
The post contains affiliate links. For more info, please visit my disclosure page <link>.
Example 3:
FTC Disclosure: I receive customer referral fees from companies mentioned in this article. All data & opinions are based on my experience as a paying customer or a consultant to a paying customer.
These are preferred formats for all kinds of FTC disclosures including clickbank products. Their intention is to disclose your affiliation to the links. Adding disclosure far away from the links or on some other page are not allowed. You should give disclosure at the beginning of the page or somewhere close to the link. Amazon might have similar practices.
