As for how agencies make money, there are a few different ways. The first as mentioned above, is a percentage of your spend. This is probably used the most and the percentage can vary greatly. Some, like myself, charge a fixed monthly fee that depends on the size and scope of your campaign. Essentially, you are charged on the number of products you are advertising, the search engines/ad networks being used and types of campaigns (search, display, shopping, remarketing). Others will use a hybrid of those compensation methods. Some may even use a commission-based approach. And of course, there can be a straight hourly approach.
The real value of an agency or account manager is to take advantage of their skills and knowledge. Yes, anybody can setup an adwords account. But can you do it correctly, efficiently and more important, profitably? Do you have the time to manage it, learn the system and to improve your campaigns? Think of it like fixing your car. Sure, you could do it. Can you do it right? Do you have the tools and the parts? Do you even want to? Sometimes it's just better, faster and cheaper to take it to a garage.
